Endoscopic radiofrequency ablation and placement of internal stents for unresectable biliary and pancreatic tumors

This study aimed to evaluate the effectiveness of endoscopic radiofrequency ablation combined with stent placement in improving biliary patency for patients with unresectable biliary and pancreatic tumors. A total of 58 patients were included in the treatment group, with different approaches based on tumor type and location. Six patients with duodenal papillary carcinoma underwent local endoscopic nipple resection followed by radiofrequency ablation of residual lesions. For the remaining 52 patients who had tumor-induced biliary stenosis, endoscopic retrograde cholangiopancreatography (ERCP) was used to assess the degree of stenosis, after which radiofrequency ablation was performed before placing a metal inner stent. As a control group, 52 patients received only single-release metal stents.

The results showed that three patients in the combination therapy group died within two months due to severe biliary tract infection and systemic failure. However, the remaining patients experienced significantly improved outcomes. The average biliary patency period was 9.2 months, and the overall survival time reached 16 months. Among these patients, 48 required repeat endoscopic procedures, with 28 undergoing additional radiofrequency ablation and 20 receiving both radiofrequency ablation and stent reinsertion. This approach extended the patency period by an average of 5.1 months compared to the control group.

In contrast, the single-release stent group had an average biliary patency of 6.1 months and an overall survival time of 13 months. These findings suggest that radiofrequency ablation plays a crucial role in preventing local tumor regrowth, thereby enhancing the longevity of the stent and improving patient survival.
